首先,我所能想到的情况两种都可以用,也不用担心别人理解上有错误,硬要说区别,可能前者（really don't）更强调我完全不知道、完全不明白、完全不理解,也就是有个not at all的含义在里面,后者则是有一定的理解但是理解不全面,
实际用起来其实都还是我不明白我不理解这样子吧

我真的完全不知道和我还不怎么明白

前者的really强调的是”不“，翻译为我真的不明白。
后者的really强调的是明白，翻译为我不是真的明白。
