annotated Reference Page and in-text (parenthetical) documentation.
8.Appendix A:Survey.
 The survey must have 7-10 appropriate questions stated objectively (without bias).
 The final survey copy must be approved (initialed) by the instructor before the survey is administered.The instructor will arrange for reproduction of the survey forms.
 Answers to each question must be provided in the survey and designed so that they are measurable (i.e.yes,no,no opinion).
 The sample surveyed must be identified and representative (fair) identified by male/female and by the age categories appropriate to the topic researched.
 The sample must include at least 30 participants.
 The survey results must be tabulated.Tabulation results should be written and graphed.A summary statement explaining the significance of each question’s results must be written.
 Surveys should appear as Appendix A of your project and should include:
1.A statement identifying the purpose of the survey and a description of the target sample
2.A statement stating and explaining the significance of the tabulated results including a graphic representation of the results of each question
3.A reflective statement evaluating the limitations of this research piece in providing information for research.
9.Appendix B:Interview.
 The interview must have at least 4 questions stated objectively (without bias).
 The instructor,before the actual interview,must approve all questions.
 The person to be interviewed must have significant information to contribute to your Public Policy Issue Project.
 Written interview notes must be taken at the time of the interview.
 A thank you note must be sent after the interview.
 Interviews should appear as Appendix B of your project and should include:
1.A statement identifying the purpose of the interview and a description of the person interviewed
2.A list of all questions asked
3.A statement stating and explaining the significance of the information obtained for each question asked and how the information obtained provided relevant information for your Public Policy Issue Project
4.A copy of your thank you note.
10.Appendix C:Demonstration of Participation
All students will submit evidence that they have participated in at least four hours of community service (real life experiences will be completed separately).Possible opportunities would include participation in the October “Make a Difference Day.” (Use Community Service/Attendance Verification Form) A participation journal needs to be completed.
