Depression
Alere’s Depression program provides support for individuals with depression,recurring depression and depressive type psychosis,excluding bipolar disorder with manic episodes,schizophrenia,psychosis and similar conditions.Following identification,members are assessed for program qualification during enrollment using the Whooley questionnaire.Participants agreeing to enroll are then further stratified for the Depression program by a registered nurse using PHQ-9 – a nine symptom depression check list for depression severity.High and moderate risk members receive scheduled telephonic call and low risk members receive education only.Similar to Alere’s MSP program,members or employees can graduate from the Depression program once they have been empowered to take charge of their depression and follow their treatment plan accordingly.
Data-Driven Interventions
At the core of Alere’s program is a rigorous,ongoing identification process,which includes predictive modeling,claims analysis,utilization managem ent,and self-referral.This allows program participants to be engaged in a meaningful experience that is individually centered,with data- driven support that uses multi-technology modalities and includes shared care (physician and case) management.Alere utilizes Impact Pro,a leading predictive modeling application,combined with proprietary risk stratification criteria to group members into different risk categories for disease management.Members will be identified and stratified on a monthly basis to capture change in risk levels in a timely manner.The key advantage of our predictive modeling and patient stratification approach is that we incorporate multiple data sources into our predictive modeling process.Members are stratified not only based on utilization markers in their medical claims data,the risk scores are also based on patient’s impactibility and gaps in care information.Alere also has the ability to add HRA data into the predictive modeling process.
Alere’s integrated health management system,Apollo,integrates with various data sources including (and not limited to):eligibility data,medical claims,pharmacy claims,laboratory,health risk assessment,daily hospitalization,direct referral,and biometric/remote monitoring data.Thro ugh our comprehensive predictive modeling and identification approach,we are better able to target specific members with the most appropriate intervention strategies.A pre-determined clinical hierarchy defines the services and the order in which they should be delivered.For example,utilization data uploaded into Alere’s clinical system provides information for a care manager to follow-up with a participant following a hospital discharge.
